It is important when seeking treatment to have a clear and concise record of symptoms. By keeping a log, you are providing your Doctor a valuable tool to more accurately diagnose and treat you. We have created two logs, two months and two weeks, with an area to write questions for your doctor, to help you with keeping track of your symptoms. Download and print the logs (.pdf format). Date the calendars and then use the coding at the bottom of each log to simplify tracking. Be sure to bring the log to your appointment to facilitate discussions with your doctor.
